IMPORTANT REMINDERS

  •  Monovalent COVID-19 vaccine supply is limited, and expiration dates are approaching. In most cases, vaccines with less than 30 days of shelf-life will not be shipped. No additional shelf-life extensions are anticipated for monovalent vaccines. It is best practice to use the manufacturer’s online expiry checking tools. The DPH does not have control over expiration dates shipped.

Placing an order is not a guarantee you will receive doses.

Quantities for direct shipment are as follows: Pfizer <5 years 100 doses, Pfizer 5-11 years 100 doses, Pfizer 12+ years 180 doses, Novavax 12+ 100 doses, Comirnaty Pfizer 12+ 180 doses , Pfizer Bivalent <5 100 doses , Bivalent Pfizer Pediatric (5-11) 100, SDV Bivalent Pfizer 12+ 50 doses , Pfizer Bivalent 12+ 180 doses, Bivalent Pfizer <5 100 doses, Moderna Bivalent 6+ 100 doses, Moderna Bivalent <6 20 doses. All other quantities will be filled via redistribution.
 You must order in multiples of the vial size (Pfizer <12 years - 10 dose vials; Pfizer 12+ years – 6 dose vials; Moderna - 10 dose vials; Novavax 12+ – 10 dose vials; Comirnaty Pfizer (12+) – 6 dose vials; Bivalent Pfizer Pediatric <5 - 10 dose vials; Bivalent Pfizer Pediatric (5-11) - 10 dose vials; Single Dose Bivalent Pfizer (12+) minimum 50, 1 dose vials; Bivalent Pfizer (12+) 6 dose vials; Bivalent Moderna <6 - 2 dose vials; Bivalent Moderna (6-11) - 5 dose vials; Bivalent Moderna (12+) - 5 dose vials.)

When EUA-labeled [Pfizer Monovalent 12+] is no longer available, BLA-labeled [Comirnaty] will replace requests.

Pfizer BLA-labeled vaccine [Comirnaty] is interchangeable with the EUA-labeled product [Monovalent Pfizer 12+] and has received the same shelf-life extension as the EUA-labeled product.

Minimum direct ship is 50 SDV - 1 dose single-dose vials.

Total Bivalent Moderna < 6 minimum direct ship is 20 doses; 2 MDV - 10 two-dose vials.

The Moderna Bivalent vaccine is the same for ages 6-11 and 12+. Orders for Moderna Bivalent should be entered in the appropriate fields for ancillary kit purposes. Just a reminder the Moderna Pediatric Bivalent is a half dose.

Providers may choose to opt out of ancillary supplies for vaccines that do not require diluent. If you order vaccine that requires diluent, you must receive an ancillary shipment to obtain diluent.

Required questions to submit form

Recording all doses: I understand it is my commitment as a COVID vaccine provider to record all doses electronically and submit them (e.g., via VAMS or CT WiZ) within 24 hours
Second dose scheduling: I understand it is my responsibility to have all patients schedule second dose appointments at the time of their first dose
Second dose completion: I understand it is my clinic’s responsibility to conduct outreach and follow-up for first dose recipients to have them complete their course on schedule at my clinic
Leadership support: The relevant clinical and organizational leaders are aware of and supportive of the plans represented within this order
I confirm that I have completed the onboarding process for the CoVP or am currently enrolled as a provider through a Federal COVID-19 Vaccine Program
